Suffolk and North East Essex ICB (SNEE ICB) have exciting opportunities for substantive posts for Registered Nurses (Adult, Mental health or Learning Disability) to coordinate NHS Continuing Healthcare assessments, case manage individuals who are eligible for NHS Continuing Healthcare and support the Discharge to Assess model. We have a number of roles available within the NHS Continuing Healthcare team which offer varied and interesting work, including undertaking NHS Continuing Healthcare assessments for people at home, in care homes and virtually, supporting personalisation through the use of Personal Health Budgets for individuals who are eligible for NHS Continuing Healthcare and wish to manage their own care package, and commissioning and case management of placements and care packages for individuals eligible for NHS Continuing Healthcare funding.
These roles are challenging but rewarding. You will rotate within the team to further develop skills and knowledge regarding NHS Continuing Healthcare and support service needs.
We have full and part time roles available, with the minimal weekly hours being 22.5hrs/week.
Main duties of the job
1. Responsible for coordinating and carrying out comprehensive assessments and making eligibility recommendations using the appropriate local and national tools as defined in the guidance for NHS CHC and NHS-FNC.
2. To collate data and evidence from comprehensive assessments to form reports.
3. Responsible for completion and review of person-centred, outcome-focused care and support plans for individuals deemed eligible for NHS funding, ensuring that the agreed package of care and support meets the individuals assessed needs.
4. To support individuals to access Personal Health Budgets to manage their care and support.
5. To carry out reviews of care and support and eligibility for NHS funding where required.
6. To support with the local resolution process where eligibility decisions have been challenged.
7. To carry out Mental Capacity Assessments and make Best Interests Decisions as appropriate, in line with relevant legislation.
8. To support with and be involved in the process of making applications to the Court of Protection or Responsible Body for authorisation of Deprivation of Liberty, including consideration of the necessity and proportionality of proposed deprivations.
9. To provide clinical advice and support to care homes and home care providers.

Qualifications
* Registered Nurse (Adult, Mental Health or Learning Disability) with active NMC Registration.
* Evidence of continuous professional development.
* Academic or professional qualification at degree level or working towards.
Experience & Knowledge
* Experience of carrying out nursing/healthcare assessments for patients with complex needs.
* Experience of person-centred care planning for patients with complex health and social care needs.
* Experience working in a complex MDT environment.
* Awareness of National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and its implementation.
* Demonstrable experience and knowledge of current key policies and priorities within the NHS and Social Care.
* An understanding of the Mental Capacity Act 2005 and its application.
* Good working knowledge of Adult Safeguarding.
* Experience in a nursing role within NHS Continuing Healthcare/NHS-funded Nursing Care.
* Experience and understanding of commissioning for complex health needs in the context of NHS Continuing Healthcare.
* High level of knowledge of National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and its implementation.
