What the Job Involves The role involves working as part of a Countywide review team, supporting adults aged 18 and over with physical disabilities, learning disabilities, acquired brain injuries, and chronic health conditions. You will conduct strengths-based assessments and reviews, develop and implement support plans, including assistive technology and home adaptations, to enhance wellbeing and independence. Your responsibilities include mental capacity act assessments, Continuing Health Care assessments, and ensuring accurate record-keeping while collaborating with colleagues, carers, and partner organizations to achieve desired outcomes. Skills Ability to apply risk enablement and evidence-based practice Ability to conduct strengths-based assessments Person-centred care plan development Excellent communication and interpersonal skills Organisational and prioritisation skills Strong IT skills for effective record-keeping Experience in inter-agency and multi-disciplinary work
