Interim PFA SEND Case Officer

Pay Rate: up to £280 per day PAYE max
Hours per week: 37 Monday – Friday, normal working hours
Role Length: This opening assignment is for 2 months

In this role, you will develop, maintain, and review EHC plans for children and young people with SEND, with a strong focus on Preparing for Adulthood (PFA). You’ll ensure transition planning starts early, helping young people and families prepare for adulthood, including employment, independent living, and community participation.
You will work with families, schools, and key agencies to create person-centred support plans, coordinating services across education, health, and care. Your focus will be on high-quality planning, assessments, and reviews, ensuring the best experience for children, families, and carers, while driving service improvements through feedback and data. Emphasis will be placed on PFA transition planning throughout each individual’s journey to adulthood.
Key Responsibilities:
Identify risks and opportunities in service delivery, especially related to PFA transitions, and escalate to management. Assess and manage risks in cases to support successful transitions into adulthood.
Contribute to the monitoring and review of services to ensure PFA outcomes are integrated into service improvement for young people transitioning to adulthood.
Provide advice and recommendations within policy to support decision-making, focusing on PFA plans that help young people prepare for employment, independent living, and community involvement.
Manage complex cases and care planning, with a focus on PFA transition support. Lead development and improvement projects to enhance transition services.
Recommend service provisions within budget to support PFA goals like employment and independent living, and guide staff on budgeting and service costs.
Build relationships with internal teams and partner agencies to ensure effective coordination of PFA services, sharing knowledge and best practices.
Work closely with service users, families, and carers to incorporate PFA goals into care plans, ensuring collaborative decision-making for successful transitions to adulthood.
Key requirements:
Educated to A level minimum / equivalent training / experience of SEN knowledge and experience, namely Code of Practice and Children and Families Act 14.
Good knowledge of Surrey’s SEND policy and practice, national guidance and legislation.
Good knowledge of SCC’s Local Offer and that of its mainstream and special schools or a willingness to acquire such knowledge.
Good knowledge of educational settings and a willingness to develop this across the 0-25 age range.
Knowledge of evidence based effective intervention and able to articulate well within report writing
Demonstrable experience of working in a collaborative way with children/young people and parents to ensure families views are heard and applied.
Significant experience of working with vulnerable children/young people, especially those with SEND
Excellent team working skills
Ability to take ownership for writing and maintaining the EHC, and to present to partnership resource allocation forums as and when required.
