Colchester Medical Practice has an opportunity for a care co-ordinator to join our team on a full-time basis for 37 hours per week. The post-holder will provide extra time, capacity and expertise to our practice team to help us to improve the care provided to our patients.

Main duties of the job
* Working with GPs and other professionals within the practice to identify and manage a caseload of patients, particularly working with vulnerable patient groups with long-term conditions that may require this extra support. This may include working with individuals, their carers, families and healthcare teams.
* Using in-depth knowledge of locally available support and services to develop care plans with patients, focusing on prevention.
* Supporting the coordination of all key activity, including access to services, advice and information, as well as ensuring that health and care planning is timely, efficient, and patient-centred.
* Helping patients prepare for, or follow-up after, clinical conversations.
* Using clinical systems on a daily basis, with confidence to run searches and keep care records up-to-date.

Person Specification
Experience
* Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups.
* Familiar with local resources and services, including how to access them.
* Motivated by helping people, with care and empathy.
* Able to deliver person-centred support in a non-judgmental way.
* Experience of completing holistic person-centred care planning assessments.
* Strong listening and communication skills.
* Diploma Level 2 in Health and Social Care, or an equivalent qualification (desirable not essential).
* Primary care experience preferred.
