We have an exciting position for a Staff Nurse to work across our clinical services in Hospice at Home and our Wellbeing day services.
The hours of the position are 30 hours, 8:30 am - 4:30 pm, working 3 days through Monday to Friday, with additional evening and weekend telephone on-call responsibilities (worked from home). These responsibilities are shared fairly amongst the qualified staff and receive a generous additional payment.
Salary: £25,708.80 per year (full-time equivalent: £32,136)
This role would entail providing compassionate, high-quality palliative and end-of-life care to patients within their own homes, and to those attending our day services including assessments, reviews, care planning, and provision of care; supporting the Team Leader in coordination of services; supervision of unqualified staff including education and training.
You will have the opportunity to be involved in the development of services, to ensure we are continually meeting the needs and expectations of our patients and their loved ones within the Rossendale Valley. We are a team who value the skills, knowledge, and experience of all staff.
The ideal candidate must have a passion and enthusiasm for excellent standards of care, be an effective communicator, and have a minimum of 2 years post registration experience, with practice in palliative/end-of-life care. A background in Community nursing would be beneficial, as would familiarity with supervision, training, and mentoring of staff.
Main duties of the job
The post holder will work across both Wellbeing day services and Hospice at Home teams delivering high standards of care to patients and their carers.
Individual patient assessment, care, and support will be integral to the role utilizing well-developed nursing skills in palliative care.
Be responsible for agreed service coordination and quality tasks, for example data collection, audits, attendance at multidisciplinary meetings.
Contribute to the supervision, education, and development of unqualified staff.
Partake in the on-call rota.
Contribute to and participate in organizational service development.

* To provide skilled nursing care under the direction of the Team Leader that is evidence-based and best practice.

* To coordinate daily activity with the Hospice at Home service.
* To undertake patient assessments in the Wellbeing setting and the patient's home, according to individual needs of the patient and their carers.
