The post holder will work as part of the Nursing component of the multi-disciplinary Assertive Outreach team. This will involve contributing to the medication management systems within the team, ordering medications on EPMA the electronic prescribing system, administering medications to the patient caseload.
The post holder will take on the role of care co-ordinator to a specified caseload, and be responsible for ensuring patients have regular CPA and medical reviews and that their needs are assessed and met within the multi-disciplinary team
As Band 5 CPN within the team you will be expected to participate in a shift pattern of working, including early and late shifts, as well as your share of weekend and bank holidays.
To form part of the Nursing component of the Multi-disciplinary Assertive Outreach Team, providing an extended hours service with the ability to respond quickly to changes in need of the teams caseload.
To attend daily handovers and maintain high levels of communication within the team and contribute to the risk management processes within the service.
To act as care co-ordinator to a prescribed caseload, arranging CPAs and medical reviews, engaging and developing a therapeutic relationship with the caseload.
