More detail about the role We have opportunities to join the Royal Devon University Healthcare NHS Foundation Trust Urgent Community Response teams of front-line staff, providing support to patients at home. This includes following discharge from hospital, enable acutely unwell patients to remain in their own home and also help with end-of-life care. Previous community experience is desired but not essential as the teams will give you support and guidance you need to excel. You will join a dynamic multi-disciplinary team, working with Health and Social Care Teams to respond to urgent situations, to enable people to stay in their own homes or ensure a timely discharge from Acute and Community Hospitals. You will be provided with a comprehensive induction programme which will include support to complete the National Care Certificate. Successful candidates will have a Level 3 Diploma / NVQ 3 in Health and Social Care or equivalent and ideally have experience of working with older people in a community setting. You will also need to be competent with IT as electronic recording is an essential part of the role. As this is a community-based position, a driving license is essential as is access to a vehicle. We may be able to offer you a development post within the team and support to undertake additional training. Urgent Community Response Support Workers are lone workers within the community, although support is always available. Working Pattern: The service operates on a 7-day rota system between 07:30-22:00, including evenings, weekends and Bank Holidays. Interview Date: To be confirmed.
