Job overview
We are looking for an experienced, motivated pharmacist to lead and support the clinical pharmacy services to acute medicine including the medical admissions unit, SDEC and Emergency Department. This post will be for a minimum of 6 months but may be extended. This is a fantastic opportunity to build upon your clinical and leadership skills within a team and a directorate that recognises the influence and impact that pharmacy has on patient care and the organisation as a whole. The department will provide the environment for you to work closely with clinicians and service managers alike to improve patient care and support the development of Quality Improvement initiatives across the Trust.
Main duties of the job
1. To be responsible for the ongoing development and evaluation of high quality clinical pharmacy services to acute medicine including ensuring an effective medicines reconciliation process and rapid turnaround of medicines at discharge.
2. To deliver, develop and evaluate training programmes for pharmacists and other members of the MDT in medicine use.
3. To deliver, develop and evaluate the Medicines Optimisation service provision in conjunction with the Clinical Services Manager, Chief Pharmacy Technician and other Lead Pharmacists.

Detailed job description and main responsibilities
1. To lead the organisation and development of the pharmacy service to acute medicine.
2. To identify and promote best practice medicines use across the relevant specialties.
3. To participate in consultant/specialist ward rounds and multidisciplinary team meetings.
4. To produce prescribing guidelines and care pathways for patients across the relevant specialties.
5. To undertake risk management review, including investigating clinical incidents and ensure compliance with medicine legislation.
6. To proactively undertake and develop Quality Improvement initiatives across the directorate and organisation.
Person specification
Experience and Qualifications
* Master Degree in Pharmacy (4yrs) or equivalent.
* Member of GPhC.
* Significant and relevant post registration hospital experience or equivalent clinical experience.
* Previous experience in service management e.g. developing guidelines and undertaking quality improvement.
