The post holder will participate as a member of themultidisciplinary team in the assessment, planning, deliveryand evaluation of patients/clients health and wellbeing needsand participate in the delivery of complex care as prescribedby the qualified practitionerTo organise and run exercise classes in the community and onvirtual platforms with the registered staff, maintaining highprofessional standards.To be able to use identified assessment tools and outcomemeasures in line with agreed competency.To have knowledge of specific physical, emotional,psychological, social, cultural and spiritual needs of theservice users and carers and, taking these into account, assistand implement appropriate action to meet those needsPlan and progress patient interventions with autonomousassessing and planning on a daily basis with indirectsupervision of qualified practitionerUnderstand a range of non-routine work procedures andpractises which require intermediate level of theoreticalknowledge and physical skills gained through relevant trainingand experienceTeach patients/carers with the indirect supervision of aqualified practitioner, and demonstrates educative support toothers, e.g. participate in induction of new staff.Demonstrate working knowledge of other providers resourceand referral systems to ensure patient needs are met, withinscope of practice.To be actively involved in team effectiveness by attendingstaff meetings and contributing as a team player to teamdevelopment. Contribute to the non-clinical day to dayrunning of the service as necessary to ensure its smoothrunning.Promote a positive image of the organisation, department andservice.Work flexibly across different locations in line with service demands.
