Then Newhaven Community Ward is the place for you! This 34-bedded community inpatient facility rehabilitates people from Brighton hospitals with a range of medical, surgical and orthopaedic conditions, often with multiple complex co-morbidities.
You will work closely with your Physiotherapy colleagues to complete comprehensive assessments, set realistic goals with patients and instigate effective rehabilitative activity to improve function and promote independence, supported by a fantastic team of Generic Therapy Assistants.
Working as part of a Multi-disciplinary team, including Flow Co-ordinators and Adult Social Care, you will help effect appropriate and timely discharges, while delivering highest standards of patient care.
The service responds to the rehabilitation needs of adults of all ages, therefore you will be required to demonstrate excellent interpersonal skills, be a great communicator and put your patients at the heart of all you do. You will have experience in and develop your skills in complex discharge planning, equipment provision, manual handling skills and knowledge of relevant equipment, home and access visits.
You should be highly motivated and be able to work autonomously as well as part of a team.
You must be HCPC registered
Main duties of the job
To work independently as an autonomous practitioner by providing specialist assessment and intervention to individuals presenting with a range of physical, cognitive, social or perceptual deficits which impact on functional ability and safety within their home environment, using an evidence based and person-centred approach.
To ensure a proactive approach to rehabilitation to maintain and improve function and to facilitate timely discharges always thinking ‘Home First’.
To work closely with Physiotherapy colleagues, as well as the wider multi-disciplinary team. This will involve providing both formal and informal clinical support where necessary.
To provide supervision and leadership for relevant junior and senior members of staff and students on practice placement.
To participate in the planning, development and evaluation of Occupational Therapy Services within a designated specialist team.
To participate in a 7-day rota including bank holiday and weekend cover as required.
Person specification
Qualifications
* Diploma/degree in Occupational Therapy or equivalent
* Current registration with the Health and Care Professions Council as an Occupational Therapist
* Evidence of recent and relevant Continuing Professional Development
* Member of RCOT
* Student educators training
Knowledge and Experience
* Knowledge of the role of a rehabilitation OT including the use of assessment, treatment and rehab interventions for Frail patients
* Varied post-graduate experience in a range of settings, including medical, surgical, older person’s rehabilitation or a closely related field
* Experience in the assessment of patients who have complex physical, psychological, emotional, sensory and social needs
* Experience in formulating SMART goals to aid rehabilitation, wellbeing and discharge planning
* Experience working as a band 6 Occupational Therapist
* Experience in providing clinical supervision formally and informally for students and staff
* Experience of working in the vacancy speciality in a previous role.
Skills
* Ability to prioritise caseloads daily
* Ability to formulate treatment goals and carry out treatment within an OT framework as well as within a multi-disciplinary framework
* To be able to clinically reason and prescribe a range of equipment to assist ADL’s
* To be able to coordinate complex discharges and follow up care
* To be able to produce clear, concise written and verbal reports
* To be able to provide leadership when the team is under pressure
* Ability to influence and effect change using a range of skills including negotiation, teaching and assertiveness
