The Community care beds facilitate early discharge from hospital and reduce hospital admissions. The CCB service delivers complex care whilst empowering patients as part of a wider multi-disciplinary team. We will work with you to develop your own personal developmental plan that supports your learning style. We provide training, clinical supervision and access to a peer support network. Community nursing in the care beds suits people who can work independently and within the wider multi-disciplinary team. You'll be resilient, motivated, and adaptable to working in an ever-changing environment and of course, a passion for providing the best possible care. You will have great communication skills and maintain a high standard of clinical work when carrying out nursing procedures. You will ensure compliance with our clinical governance framework, motivate colleagues, champion service user safety and dignity, and drive continuous improvement.
Main duties of the job
The post holder will be responsible for their designated workload within their specialty area with access to the support, advice and management from a senior clinician whilst not directly supervised. In doing so, they will assess, plan, deliver and evaluate patient centred programmes of care ensuring that they practice within their sphere of competence and knowledge. The post holder will work to professional protocols and regulatory body codes, standards and guidance at all times ensuring that their practice is grounded in the best available evidence based theoretical and practical knowledge.
