Job summary
We are looking for an Advanced Clinical Practitioner to join our team, to help deliver high-quality, person-centred healthcare to residents in care homes across Kendal.
Your role supports the care of residents in the care homes aligned to the practice, where your skills and passion for the care of frail and older people will be highly valued.
Based with the James Cochrane Practice you will work closely with lead care home clinicians in the practice and Kendal PCN to care for patients in local Care Homes.
Main duties of the job
1. Provide expert clinical care and management of frail and older adults in care homes.
2. Utilise prescriber qualifications to ensure optimal medication usage and management.
3. Collaborate and work closely with a well-established Care Home multidisciplinary team and our neighbouring GP practices in Kendal to deliver holistic patient care.
4. Support and advise patients, families, and carers with your clinical expertise.
5. Promote and implement best practices in care of older adults, ensuring high standards of dignity and respect.
6. Recognition and management of palliative and end of life situations.
7. Support best practice by working closely with the individual, their carers/family/NOK and communicate well with health and social care teams to ensure we consistently deliver high quality personalised care.
About us
We are a 2-site, part-dispensing practice in Kendal with around 17,500 patients, keen to attract caring, motivated, and passionate colleagues. We are a training practice with an active frailty and care home support team. We offer a wide range of enhanced services including minor surgery, women's health and menopause, coil/implant fitting, as well as Dermatology and ENT GPwER clinics.
We strive for a good work/life balance while achieving high-quality patient care and offer a fair allocation of work across all the GP team. We have a Quality Improvement culture and continually develop our systems to respond to the changing environment of primary care, aiming to minimise administration workload and maximise job satisfaction to the benefit of our patients and our team.

Care, compassion and commitment to looking after frail and older adults well are essential qualities we expect from the post holder.
Continuity is key to safe, effective, and person-centred care, so establishing productive professional relationships with care home staff and management, whilst getting to know the residents over time will support the delivery of excellent care in acute, long-term, and palliative conditions.
The role involves remote working, in care homes in the Kendal area. It may on occasion involve some out of hours or flexible working to support patient need.

Essential
* Experience of working in frailty and the care of older adults.
* Working autonomously in an advanced clinical practitioner role.
* Managing both acute care needs and long-term health problems relevant to frail and older adults, including palliative and end of life care.
* Prescribing.
* Audit and project work.
* Research.
Desirable
* Working with community development initiatives.
* Health needs assessment.
* Experience of working within a primary care or General Practice.
* Experience of case management.
Qualifications
Essential
* Registered first level nurse MSc or equivalent.
* Relevant nursing/health degree.
* Mentor/teaching qualification.
* Clinical supervision training.
* Independent prescribing qualification.
