Role Overview:
Thirteen Group is on the lookout for 2x Assessment & Support Coordinators to join our project which will support government ambition to reduce veteran homelessness and rough sleeping by delivering housing support interventions. Thirteen has submitted an application to secure funding from the Armed Forces Covenant Fund Trust. Successful grants will be awarded on behalf of the Office for Veterans' affairs (OVA) and if successful, this project will commence from 02 June 2025.
This is a roam/hybrid role, so you’ll work mainly from home and at least one day a week from our fantastic office at Hudson Quay in Middlesbrough. We’ll need you to be flexible around business and team need, so you may need to come into the office more frequently when required.

Key Responsibilities:
* Support and empower clients in a person-centred manner.
* Coordinate referrals entering the service.
* Undertake, authorise and review individual client need assessments, support plans and risk assessments
* Deliver a high quality standard of support which promotes independence.
About You:
* You will have experience of supporting homeless people and/or lived experience as a Veteran, understanding the issues faced
* You will need to have an understanding of safeguarding.
* You will need a full UK driving licence and access to a vehicle.
