The Reablement Service is a dedicated team whose main goal is to enrich the lives of those returning home from hospital to ensure they maintain their independence and healthy living in a safe manner within the comfort of their own home. We’re responsible for the planning and programming of calls for both the Reablement Service and the Integrated Crisis Response Service, using the Electronic Rostering and Monitoring system to provide timely performance data in terms of outcomes, unit cost, and the quality of service provision.
As a Reablement Co-ordinator, you’ll join a team of over 80, including front-line staff, reablement assistants, officers, co-ordinators, and administrative employees.
This post is full time (37 hours) and permanent. You’ll be based at the Neville Centre on the Leicester General Hospital site, working a mixture of morning (7.30am to 4.00pm) and evening shifts (1.45pm to 10.15pm) across a rota covering 7 days a week, including weekends, bank holidays, and other public holidays. Part-time hours may be considered for between 18 and 37 hours for the right candidate.
What you’ll be doing
As a Reablement Coordinator, you’ll be instrumental in planning all support visits within the Reablement Service across the Leicester area to help people remain independent in their own homes, avoid hospital admissions, and live with respect and dignity.
Day-to-day activities will include:
1. Shift handover to colleagues, clarifying outstanding visits with residents.
2. Preparing to take phone calls from staff and the public.
3. Tracking requests and actions via email, responding within 2 hours.
4. Entering care plans on the Webroster (Electronic Rostering System) and allocating visits.
5. Communicating with co-workers and front-line staff regarding visits, ensuring timely and accurate recording.
6. Sending feedback reports to allocated case responsible staff regarding cancellations or other visit issues.
7. Receiving all incoming referrals into the Reablement and Integrated Crisis Response Service.
8. Facilitating hospital discharges through a strengths-based approach as part of the Home First Locality Decisions Unit.
What you will need
As a Reablement Co-ordinator, you’ll need to be disciplined, possess excellent organization and IT skills, and be able to prioritize your workload under pressure. Additionally, you should have:
1. Great interpersonal skills to communicate effectively with a diverse range of people.
2. The ability to adapt your approach to meet the needs of individuals.
3. Previous coordination experience within a social care setting (desirable).
4. A willingness to learn and develop new skills quickly.
