This post will provide and support expert and effective social work practice in complex situations, assessing and managing high levels of risk and providing expert opinion in relation to decisions surrounding complex cases. You will undertake a professional leadership role taking responsibility for the learning and development of others, through leadership and management, supervision, mentoring, assessing and teaching.
You must be a qualified social worker and be registered with Social Work England. Flexible working and full or part time hours considered.
The Locality teamwork in partnership with individuals requiring longer term adult social care support. Referrals to the team come directly from the See & Solve Community team, for ongoing support following Active Recovery and/or after a hospital discharge package has been put in place. The team undertake Care Act assessments to determine eligibility and identify strengths, goals and needs that may require statutory support. We work jointly, between Social Work and Occupational Therapy as well as with Sensory and Telecare colleagues, to identify ways in which an individual can maintain their independence. The team also ensure individuals receive timely and proportionate reviews ensuring that an individual’s goals and support needs are reflective of their changing circumstances. The team works with a diverse client group and promotes the core Social Work values of respect, inclusivity, anti-oppressive practice, self-determination, choice and control. When necessary, we undertake court applications to the Court of Protection to ensure people are kept safe and protected from the risk harm.
