The Hospice at Home Team is part of the Community Specialist Palliative Care Team across Northamptonshire. It is a county-wide service providing holistic nursing care for end-of-life patients in their own home, over a 24-hour period. We provide nursing care to patients who require symptom management and end-of-life care at home. Excellent communication and interpersonal skills are essential, as you will be required to communicate sensitively and compassionately with patients who are approaching the end of their life and their families.
Experience of working within the community setting and palliative nursing would be ideal and a passion to provide outstanding holistic nursing care is essential.
This 24-hour service requires you to be flexible to a varying shift pattern, which will include early, late, and night shifts, as well as rotational weekend and bank holiday working. This post requires travel across Northamptonshire to patients' homes as required.
Main duties of the job
The post holder will have an important part to play in providing holistic care and meeting the specific needs of these patients, their families, and those important to them without direct supervision. This will include:
* Assessment of needs and symptoms and implementation of individualised care. Giving appropriate information to their families and involving them in developing the plan of care and its ongoing evaluation.
* Administer prescribed medication, as required.
* Ensure accurate records are maintained.
* Ensure a coordinated team approach by using effective communication within the team and other community services involved.
* Providing sensitive information and support to those closest to the patient.
We are looking for a nurse who has personal resilience and can manage the care required for patients and their loved ones who are experiencing emotions expected of those in their last short weeks of life.
The post holder will be expected to assist in the learning and development of students and clinicians.

Qualifications and Experience
* Palliative Care Qualification
* Experience of Assessing and Lead supervisor role to Pre reg students
Experience
* Experience of working within a multi-professional team
* Community experience
* Experience of communicating with patients and their families approaching the end-of-life
* Experience of assessing patient symptoms and giving anticipatory medicines
* Experience of preparing and administering medicines via a syringe pump
* One year post-registration experience
* Audit involvement
* Evidence of understanding of Specialist Palliative Care
Knowledge and Skills
* Understanding of clinical governance and quality issues evidence-based practice
* Demonstrates continued professional development other than mandatory training
* An understanding of holistic nursing care
* Knowledge of symptom management for patients approaching the end of life
* Knowledge of National and local NHS policies in Specialist Palliative Care
* Knowledge of clinical supervision and reflective practice
