Job overview
We are currently recruiting for a Support Worker to join our Intermediate Care Team on a full-time permanent contract working 37.5 hours per week. The Moor to Sea Intermediate Care Team is based at Totnes Hospital, although we cover a large geographical area.
As a Support Worker for Intermediate Care Team (SWIC), you will assist Occupational Therapists, Physiotherapists, and Community Nurses working under supervision in providing an Intermediate Care Service to patients to promote faster recovery from illness, prevent unnecessary acute hospital admissions, support timely discharge, and maximise independent living within the recommended time frame of six to eight weeks.

* On a daily basis, you can make a difference by helping patients through recovery and returning back to health and independence.
* You will interact and communicate with patients, carers, and relatives appropriately and effectively, ensuring information is understood by all concerned while respecting individual’s privacy, dignity, wishes, and beliefs.

Detailed job description and main responsibilities
* Communicate with a range of people on matters appropriate to them and the situation.
* Collect, collate, and record appropriate observations and report any significant changes that might be detrimental to the patient.
* Work independently using a wide range of skills.
* Teach patients, carers, and relatives the correct use of specialist equipment and daily living aids.
* Assist in developing and restoring the function of the body using exercise and equipment for the patient to gain an optimum level of mobility.
* Deliver nursing procedures, e.g. simple wound management, venipuncture, capillary blood sugar monitoring, and any other procedure deemed appropriate during the intermediate care period.
Ability to travel efficiently around the Bay is important to carry out your duties (e.g. Full driving licence and vehicle).
Person specification
Qualifications and training
* NVQ level 3 in Health & Social Care or equivalent qualification/experience
* Care certificate
* English & Mathematics at GCSE grade A-C (9-4) or level 2 functional skills
Knowledge and experience
* Previous experience in a care environment or able to demonstrate a caring nature/life experience
* Understands the need for strict confidentiality
* Working knowledge of complex care needs of some individuals/families including Safeguarding Children & Adults, Domestic Abuse, and Mental Health
Specific skills
* Basic computer/keyboard skills
* Fine motor skills for venipuncture, phlebotomy, and other clinical tasks
* Good interpersonal and communication skills
* Able to prioritise and organise work
