We're committed to helping every person we support feel safe, happy, empowered enabling them to live the best life possible.\n\ntime\n\nWe provide a psychology led multi-disciplinary treatment model to deliver a high quality, specialist service for people who have a neuro disability or acquired brain injury.
\n\nWhat you'll be working:\n\nFull time\n\n38.5hrs per week\n\nDays and nights flexible\n\nWeekend working on a rota\n\nWe are Hiring for 8 Senior Support Worker's\n\nWhat you'll be doing:\n\nSupport individual service user needs in accordance with the Person-Centered Plan (PCP).\n\nLead by example, demonstrating best practices in care delivery, communication, and professional conduct.\n\nSupervise and guide support workers during shifts, providing on-the-job coaching, feedback, and support.\n\nAct as the primary point of contact during shifts for support workers, managing any incidents, queries, or emergencies as they arise.\n\nAssist in the conduct of regular audits of service user documentation, medication administration, and care procedures, working under the guidance and supervision of management.\n\nAdminister medication in line with training and company protocols, ensuring accurate record-keeping and adherence to safety standards. (Hazelwood Unit Only)\n\nAssist with aspects of personal and intimate care as required for each service user which may include:\n\nBathing, showering, drying, shaving\n\nDressing, undressing\n\nHair/nail and foot care\n\nAssisting to eat\n\nDomestic and clinical waste disposal\n\nDental hygiene\n\nToileting/catheters\n\nStomas/PEG feeds\n\nSupport service users with physical disabilities using trained techniques and aids e.g. hoists, wheelchairs, modified bathing equipment.\n\nPromote service users' rights to confidentiality of information in accordance with the guidelines of company policy and data protection laws.\n\nActing as a representative of Active Care Group at all times when liaising with external stakeholders including medical professionals, care managers, families, advocates, therapists, prospective service users etc.\n\nBe assigned to service users as a key worker.\n\nResponsible for ensuring individual service user documentation is accurate and up-to-date at all times.\n\nObserve and operate at all times in accordance with the Company's policies and procedures.\n\nMaintain a flexible approach in relation to the working pattern to ensure the smooth operation of the service including working across sites, additional hours, weekends and public holidays.\n\nResponsible for attending and completing all mandatory training.\n\nWhat you'll have:\n\nNumeracy and literacy Level 2\n\nLevel 3 in Health and Social Care or willingness to undertake \n\nProven experience in a support role.\n\nSkilled in communication, providing clear instructions, and maintaining team morale.\n\nCompassionate and Caring\n\nAbility to work as part of a team and as an individual.\n\nSelf-confidence and ability to manage challenging situations.\n\nIT literate.\n\nStrong organisational, time management, planning and communication skills.\n\nProven reliability in terms of full attendance and timekeeping.\n\nCompetencies:\n\nService user focus\n\nLearning and development\n\nHealth and safety\n\nManaging and planning\n\nCompany standards\n\nTeam work\n\nCommunication\n\nMedical\n\nThis job description is intended to provide an outline of the role at present.
