4Recruitment Services are seeking a Deaf/Deaf Blind & Hearing Impairment Officer to assist our client based in Harrow.
The post holder is required to:
* manage a caseload of deaf / deafblind citizens, requiring a holistic assessment in compliance with the Care Act 2014.
* ensure the identification of needs and outcomes to meet support needs and to enable deaf/deafblind citizens and those with a hearing impairment to maintain and develop their independence.
* provide and support interventions to help citizens to develop and maintain independent living skills and encourage community involvement.
* provide support to citizens to identify and commission resources that enable them to choose the best options that fit their assessed need and preferred lifestyle choices.
* work 21 hours per week Monday Tuesday & Wednesday.
DUTIES & RESPONSIBILITIES INCLUDE:
* Undertaking specialist deaf/deafblind/hearing impairment assessments (including for appropriate environmental aids, and assistive technologies), and identify current needs and outcomes in compliance with the Care Act 2014.
* Undertake in conjunction with a citizen, a holistic overview assessment of a citizens needs, abilities and circumstances to determine the level of resource, equipment or minor adaptations required to optimise the citizens’ independence.
* Identify a citizen’s eligible needs in compliance with the Care Act 2014.
* To monitor and review care packages as necessary.
* Provide and assist on a range of issues in relation to a citizens support needs including detailing information on risks / risk management, benefit maximisation, support services and equipment to inform the range of choices available when a citizen comes to develop their package of support.
* Support and help to develop and maintain a citizens’ independence through achieving for themselves and working towards minimising levels of dependency.
* Provide assistance to citizens to develop and commission cost effective care packages of support, and purchase and provide equipment to meet their requirements and needs and obtain authorisation from the relevant budget holder.
* Liaise with providers and other agencies to help arrange identified services on behalf of the citizen.
* Identify, including through the review process, any safeguarding and quality of care issues and initiate action to ensure vulnerable adults are protected in the community and that the quality and standard of services provided are at, and beyond, the level that would be anticipated.
* Undertake capacity risk assessments in conjunction with other professionals (e.g., when supporting citizens to continue living in the community or assessing residential care options).
* Supporting deaf and deafblind citizens with mental health issues.
* Provide advice and information -for example through a drop- in service / deaf forum etc.
ESSENTIAL REQUIREMENTS:
* Front-line experience with Deaf/Deafblind adults.
* Broad experience with social care needs, including learning disabilities, physical disabilities, older persons, and blind individuals.
* Experience working in a multi-agency environment/partnership.
* Experience interviewing service users and carers.
* Appropriate qualification, e.g., DipSW (desirable).
* British Sign Language L3 (essential).
* Deafblind L3 (essential).
* Computer literacy.
* Ability to communicate in Deaf/Deafblind manual language.
