Key Responsibilities: Comprehensive Assessments: Take responsibility for conducting thorough assessments of needs and risks for individuals referred to the service. Care Planning: Plan and provide care, support, and signposting for service users, ensuring smooth onward referral where necessary. Collaboration: Work closely with service users, colleagues, and other services/agencies to create tailored plans to meet individual needs. Service Development: Contribute to the ongoing improvement of the service and the development of professional practices. Equality & Professionalism: Uphold the Trust's Equality's Framework, ensuring professional relationships and behaviour at all times. Effective Communication: Maintain sensitive, confidential, and professional communication in all interactions. Multi-Agency Liaison: Build strong working relationships with Trust teams, Social Care services, Police, Safeguarding, and third-sector organisations to achieve the best outcomes for service users. What We&x2019;re Looking For: Qualified mental health nurse with Band 6 experience or equivalent. Strong assessment, planning, and collaborative working skills. Flexibility to work various shifts, including weekends and nights, in a 24/7 service. Excellent communication and teamwork abilities.
