Job overview
The post holder will be responsible for ensuring the highest standard of clinical care is provided for patients referred to the team for District Nursing Services delivered over a 24-hour period.
In pursuing these duties, the post holder will ensure compliance with the NHS Constitution.
Main duties of the job
MAIN DUTIES/KEY RESULT AREAS:
The post holder will be responsible for ensuring the highest standard of clinical care is provided for patients referred to the team for Palliative Care Service. The post holder will support the team leader in the provision of strong clinical leadership and in ensuring the principles of clinical governance are implemented at a local level within the designated team.

Knowledge
* Understanding of the NHS Constitution and its implications
* Registered Nurse
* Significant experience and competence in community nursing
* Able to assess patients with complex health care needs in community settings and develop care plans to meet needs.
* Experience of risk management and governance.
* Knowledge/experience of working in a multi-cultural community
* Teaching & Assessment Qualification (ie ENB 998)
* Commitment to clinical supervision
* Extended/Supplementary Nurse Prescriber
* Care and Management of PICC and Hickman Lines
* IV Therapy Administration
* Verification of death
Communication
* Ability to maintain accurate, timely, and contemporaneous records
* Excellent interpersonal skills
* Experience of SystmOne use
