Job summary It is envisaged that the post holder will work 10 programmed activities. Following appointment there will be a meeting at no later than three months with the Lead Consultant to review and revise the job plan and objectives of the post holder. Main duties of the job Provide direct clinical care to patients (assessment, investigation, diagnosis and management) including the assessment and management of risks. Deliver evidence-based care. Be committed to multidisciplinary care and support the team in organising the care of patients, providing care packages, advising on appropriate interventions. Engage with service transformation programmes including attendance at consultant meetings for their service and directorate. Work closely with other teams (and consultants) including the primary care team, HBT, inpatients, liaison, EIS and other CMHTs to provide safe care across points of transition. Work with colleagues in the community service to provide consultation and liaison with general practitioners and other referrers as necessary. Provide expert advice and input in relation to the care of patients requiring detention under The Mental Health Act Undertake Mental Health Act assessments in conjunction with team colleagues.
