Job summary Ward Two is currently looking to recruit a proactive and patient centred Discharge Coordinator. As a ward we prides ourselves on our effective team work. As a discharge Coordinator on Ward Two, you will be required to interact closely with all members of the Multi-Disciplinary Team and other external health and social care services in order to facilitate safe and effective discharges for our respiratory patients.
