Job summary The primary role of the Advanced Clinical Practitioner for Clozapine & SMI Health Checks will have operational and clinical leadership for the delivery of the clozapine pathway and delivery of SMI health checks. The post holder will demonstrate a high level of expertise within the specialist service providing advice, education and support to staff, patients, their families and carers. The role has the following priorities: To practise autonomously as an advanced clinical practitioner and non-medical prescriber within the clozapine and physical health (SMI) speciality area to provide patient-centred clinical care. This will encompass the skills of advanced clinical assessment, examination, diagnosis and treatment within an agreed scope of practice throughout the directorate. Supporting new ways of working that emphasises a more efficient and patient focused service, and will ensure the safe treatment, referral and discharge of patients. Responsibility for leading the on-going development of clinical practice and standards of care within the clozapine service in collaboration with the lead pharmacist, including the development of policies, procedures, protocols and guidelines in collaboration with multidisciplinary colleagues. Delivering a 20% clinical component and 20% related to appraisal, clinical audit, teaching, continuous professional development and research, and 60% operational leadership for the Clozapine service Main duties of the job The main duties of this post are to: Contribute to the delivery of specialist interventions, focusing on clozapine & physical health/wellbeing Deliver medicines optimisation in collaboration with the lead pharmacist for clozapine, using a person-values centred approach.
