Job summary
Cantilupe ward is a ten bedded acute inpatient ward for older adults with organic mental health problems, requiring inpatientcare and treatment. Patients requiringadmission often have complex physical co morbidities and applicants should havean interest in both mental and physical health care delivery. We also haveclose links with the Hospice and receive training to enable End of Life Care tobe delivered in the ward when appropriate.
The ward has a multi-disciplinary teamapproach to deliver holistic care, including nurses, physiotherapists,occupational therapist, activity support workers, psychiatry consultants andjunior doctors. You will also work alongside colleagues in Adult Social Care .Cantilupewelcomes and supports student nurses as part of our team. When you join the older adult mental healthservice in Herefordshire and Worcestershire you will become part of a largecaring and compassionate team. Everyone receives a warm welcome, a supportiveand needs led induction, ongoing training, and flexible working patterns. Ourmanagers carry clinical caseloads to ensure they lead both operationally andclinically and we encourage a coaching culture of development across our teams.
Cantilupeward operates a rotational shift pattern system over a 24 hours / seven dayperiod.. The day starts at 0730hrs till2000hrs and the night shift at 1930hrs till 0800 hrs The post is full time butpart time hours would be considered.
Main duties of the job
Your day will commence with clinical handoverfrom outgoing shift and you will then plan the day ahead with your team. Thiswill include allocation of patient care and therapeutic activities. Ensuring all activities of daily living aresupported in a person centred way focussing on independence and wellbeing.Medication administration, liaising with the medical and therapy team, careplanning, risk assessments, discharge planning, MDT meetings and ensuring theMental Health Act is correctly administered.You will supervise health caresupport workers and junior colleagues.
