The following represent a broad outline of tasks the post holder will be expected to perform in achieving their key result areas. These are not intended as a fixed list of duties. The post holder will, under the direction of the Chief Pharmacist, Senior Pharmacist and Senior Technicians, perform whatever duties are reasonably necessary to achieve the job purpose and the overall safe provision of pharmacy services throughout the Trust: The underlined are not essential for the Band 4 progressive role and training will be given once in post To work closely with the Senior Pharmacy Technicians, Lead Pharmacists and pharmacy team in providing a medicine management service to the wards To obtain accurate medicine reconciliations in a timely manner from service users/carers/care providers and identify and inform the pharmacist of any anomalies found. Document medicine reconciliation onto JAC and Insight To identify, assess and check suitability of patients own drugs for administration use while an inpatient and on discharge To highlight the need of any additional compliance aids and supply if necessary, to assist with medication compliance To liaise with GPs, community pharmacies and other primary care professionals to ensure safe use of medicines and continuity of care is maintained To participate in discharge planning with other multidisciplinary teams ensuring medications are prescribed efficiently To work on various wards/areas to provide pharmacy services including, assessing, checking and re-using patient own drugs, giving advice to staff and service users, providing information on local guidelines and policies and ensuring they remain up to date To engage and participate in discharge planning and dispensing of prescriptions in a timely manner to ensure medicines are available for the service user to take home and provide any medication counselling if needed To order any medicines required for service users and ward stocks To organise and participate in quarterly controlled drug checks at ward level and return any unused or expired drugs back to pharmacy following the relevant SOPs Assist in any relevant audits in accordance with clinical governance requirements Assist in training newly appointed medicine management technicians Participate in training of ward and development of ward staff on efficient systems of ordering, EPMA To regularly top up stocks to wards and complete regular checks To maintain and plan own CPD to maintain skills and knowledge required by GPHC Other Duties Dispensing prescriptions e.g. inpatient, outpatient. To work as an accredited accuracy checking technician after obtaining the necessary competency training and qualification. To participate in efficient stock control and stock management systems To assist in the dispensary and cover for other staff members Participate in departmental systems of supervision and peer review Assist in the provision ofother routine supplies to external units To be able to order and support the procuring of medicines Participate in bank holiday, late night, early morning and weekend rotas as needed To deliver training presentations as part of a team to department and ward staff To respond to queries and information requests Undergo further training as necessary and attend regular progress reviews Participate in regular supervision Attend all mandatory training
