Job summary
Are you a pharmacist looking to work in a GP practice? This role includes working in a clinical role in our GP practice to support medicines management and improve patient care. This will include reviewing repeat prescriptions, reauthorisations, undertaking medication reviews, and monitoring high-risk patients to ensure safe and cost-effective prescribing.
You will be based in Bryntirion surgery working as part of a multidisciplinary team. There is professional support and engagement with the ABUHB medicines management team, and you can join the ABUHB network of practice pharmacists.
Main duties of the job
* Improve the quality and effectiveness of medicines management within GP practices
* Provide patient-focused, prudent healthcare
* Promote medicines safety and review polypharmacy
* Develop the practice pharmacist role, becoming a core member of the practice team working effectively with other health and social care professionals delivering services to patients
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

* Registered with GPhC
