Main duties of the job
You will deliver medicines optimisation services, and undertake clinical medication reviews to proactively manage people with complex polypharmacy. Reviews are both face to face and on the telephone. In addition, you will support the practice in providing expert advice with medication queries and monitoring.

Witterings Medical Centre have over 10,000 patients with 4 partners, 3 salaried GPs, an experienced nursing team and an excellent administrative team. We are one of 8 practices in the Chichester Alliance of Medical Practices Primary Care Network.
Job responsibilities
-Contribute to the delivery of effective dailyprocesses for the transfer of care between hospitals and GPs includingeffective signposting, recommendation of medication changes and communicationaccordingly.
-Support colleagues and patients with medicationqueries and monitoring.
-Use structured medication reviews to supportpatients to take their medications, to get the best from them, reduce waste andpromote self-care.
-Undertake clinical medication reviews toproactively manage people with complex polypharmacy.
-Provide leadership on person-centred medicinesoptimisation (including ensuring prescribers in the practice conserveantibiotics in line with local antimicrobial stewardship guidance) and qualityimprovement.
-Take a central role in the clinical aspects ofshared care protocols, clinical research with medicines, liaison withspecialist pharmacists, liaison with community pharmacists and anticoagulationclinics.
-Contribute to the General Practice quality andoutcomes framework (QOF) and enhanced services.
-Specifically, contribute to the QualityImprovement Schemes (QIS) for practices (e.g. to reduce prescribing spend andunplanned admissions).
-Work with General Practice teams to ensure thatpractices are compliant with CQC standards where medicines are involved.
-Take a lead role in some aspects of supportingfurther integration of General Practice with other healthcare teams to helpimprove patient outcomes, ensure better access to healthcare and help managegeneral practice workload.
-Develop relationships and work closely withother pharmacy professionals across Primary Care Networks and the wider healthand social care system.
-Take responsibility for the care management ofpatients with some of the chronic diseases.
-Contribute pharmaceutical advice for thedevelopment and implementation of new services that have medicinal components(e.g. advice on treatment pathways and patient information leaflets).
-Be prescribers, or committed to completingtraining to become prescribers.

Person Specification
Qualifications
* Undergraduate degree in pharmacy and registration with the GPC.
Experience
* Minimum 2 years experience as a pharmacist.
* Working towards independent prescribing qualification.
* Experience of common acute and long-term conditions.
* Works effectively independently.
* Excellent interpersonal, influencing and negotiating skills.
* Rigorous in following legal, ethical, professional and organisational policies and codes of conduct.
* Committed to continuous professional development.
* Ability to apply research evidence base into the workplace.
* Excellent organisational skills.
* IT literate.
* More than 2 years experience as a pharmacist.
* Independent prescriber.
* Experience of medication reviews relevant in General Practice.
* Track record of improving work processes.
* Previous experience of working in a GP surgery.
* Up to date knowledge of CQC regulations in General Practice.
* Good understanding and history of quality improvement work through audit.
* User of TPP SystmOne.
