We are looking for a highly organised, motivated and enthusiastic individual to work within the Osteoporosis team at Mansfield Community Hospital, 22.5 hours a week. This is a fantastic opportunity to be part of a great team who take pride in patient care.
You will provide comprehensive administrative support to the Osteoporosis Specialist Nurses based at Mansfield Community, delivering Bone Mineral Density Scans and Outpatient Treatments to patients in the surrounding area.
You will need to be computer literate and able to perform well under pressure. You will work alongside the existing Patient Pathway Coordinators in booking patient treatments, appointments, distributing, filing and scanning of referrals and reports.
We are looking for an enthusiastic and motivated team player with empathy, compassion, excellent communication and good problem-solving skills. You will require high levels of tact and diplomacy with the ability to prioritise your own workload effectively to enable day-to-day management.
You will be working with a highly efficient team of Patient Pathway Co-ordinators, Clinical Typists and Records Assistants. The role of a Patient Pathway Coordinator is to provide high-quality administrative support to the Osteoporosis Service.
On a day to day basis this would involve:
* General admin, receiving and managing incoming post and referrals and distributing them to the Consultants and nurses.
* Dealing with phone calls, emails and face-to-face queries from Patients and Colleagues.
* Managing the Patient tracking list to ensure all patients are treated in a timely manner and adhering to National operating standards.
* Booking patient appointments.
* Audio typing.
* Managing consultant diaries and annual leave, cancelling clinics when needed.
To be the main point of reference for the Consultant to ensure administrative support throughout the patient’s journey at the hospital, working as a team with Patient Pathway Co-ordinator colleagues to ensure appropriate cover.
Ensure that all appointments across the patient pathway from referral to discharge are booked in line with targets/standards. Undertake validation of the RTT Patient Tracking List.
Investigate and take appropriate action where pathways are incomplete to ensure that patients are receiving timely treatment and Trust information is robust following agreed escalation procedures.
Confirm in writing, details of the agreed appointment dates, thus ensuring patients have all the information required for their outpatient visit.
Change and cancel Clinic appointments as appropriate and in accordance with the Trust Access Policy.
Liaise with Clinic Prep Clerk / Records Assistant as appropriate to ensure all case notes are available and fully prepared for the patient’s attendance at Clinic.
Ensure all Clinics are fully reconciled in conjunction with the Clinic receptionists. All outcomes should be actioned within 24 hours in line with Trust SOP and protocols.
