The role
The Pathway Worker is instrumental in supporting residents of Durham SHAP to achieve independence by developing tailored Learning and Development Plans. These plans encompass training, work experience, employability activities, and job search support to empower residents to access education, training, and employment opportunities.
Key responsibilities include assessing residents’ independent living skills, coordinating practical support for housing transitions, and facilitating personal development activities such as financial literacy, cooking, and wellbeing initiatives. The Pathway Worker will also cultivate relationships with employers, training providers, and external agencies to support residents’ pathways.
This role demands strong communication and organisational skills, a commitment to safeguarding, and the ability to build person-centred plans to meet diverse needs. Working closely with the Service Manager, colleagues, and external partners, the Pathway Worker ensures residents are equipped with the skills and confidence needed for long-term stability and reintegration into the community.

Emmaus North East is proud to deliver the SHAP service, on behalf of Durham County Council (DCC). This innovative service responds to the pressing need for a holistic support model that provides a pathway of provision for individuals experiencing homelessness.
SHAP is designed to offer flexible, person-centred support alongside appropriate accommodation. Its ultimate goal is to guide individuals towards independent living. The service addresses gaps in existing provision, ensuring that those who might otherwise be left without suitable support can access the tailored assistance they need.
