Working closely with GPs, community services, and social care teams, you will ensure effective case management and promote patient independence.The work is challenging, interesting and hugely varied and the ideal candidate will have a strong team work ethic and sense of accountability, accompanied by the ability to work well under pressure and manage multiple tasks simultaneously.Experience of effective team management, including dealing with conflict is essential and forms an integral part of this role. The post covers the North East Essex area so a full UK driving licence and access to a vehicle for work is essential.
