Support Practitioner

Job Summary:
We are currently recruiting Support Practitioners to join our established teams in the Scottish Boarders. In this role, you will provide support to individuals with diverse needs, including those with mental health issues, autism, physical disabilities, and learning disabilities.
Key Responsibilities:
1. Working with individuals who need support for a range of reasons, such as mental health, autism, physical disabilities, and learning disabilities, by utilising a person-centered approach
2. You will support people with all aspects of daily living at home and in their communities
3. Updating care plans which you should create, update, and review frequently to make sure they are always up to date and based on recent developments and feedback from the appropriate parties.
4. Provide suggestions for changes, improvements, and inventions that could give individuals greater choices and enable them to build social networks by applying your problem-solving and creative thinking abilities.
5. Effectively interact by creating and maintaining professional connections with friends, family, and colleagues.
6. To guarantee every individual has access to the appropriate information at the appropriate time to make informed decisions, comply with existing human rights-based policies and processes.
7. Cooperate as a team and work independently,
