JOB SUMMARY The Clinical Pharmacist will have a key role in supporting delivery of the Network Contract DES Service specifications. For the new Structured Medications Review and Optimisation requirements this will include tackling over-medication of patients, including inappropriate use of antibiotics, withdrawing medicines no longer needed through NHS-led programmes such as low priority prescribing and medicines optimisation more widely. For Enhanced Health in Care Homes residents will benefit from regular clinical pharmacy-led medicines reviews. DUTIES AND RESPONSIBILITIES OF THE POST The Clinical Pharmacist will: Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas. Act as a lead for repeat prescribing at the practice Hold a prescribing qualification Assist with routine and urgent prescription requests throughout the day Take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex poly-pharmacy, especially the elderly, people in care homes, those with multiple long term conditions (in particular COPD and asthma) and people with learning disabilities or autism (through STOMP - Stop Over Medication Programme). Support the PCN with delivering prescribing aspects of the Investment and Impact Fund Provide support and oversight to practice repeat prescribing systems and medicines reconciliation. Support the implementation of MHRA and other safety alerts at practice level. Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, while contributing to the quality and outcomes framework and enhanced services. Through structured medication reviews, support patients to take their medications to get the best from them, reduce waste and promote self-care. Support further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload. Develop relationships and work closely with other pharmacists across networks and the wider health system, particularly the wider DHC and PCN Clinical Pharmacy team. Take a central role in the clinical aspects of shared care protocols, clinical research with medicines, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties) and anticoagulation. Work with practices to support CQC compliance. Provide specialist expertise in the use of medicines while helping to address both the public health and social care needs of patients in the network. INDIVIDUAL RESPONSIBILITIES Adhere to Practice policies and procedures and relevant legislation including the requirements of any professional bodies and attend/undertake all mandatory and other training as identified and agreed by the practice. Participate in reflection, self-evaluation and continuous professional development including performance review. Ensure mandatory training is up-to-date at all times. Maintain a good working knowledge of health and safety procedures. Promote client involvement in the management of the service. Contribute to ongoing projects as required. Maintain administration systems and workspaces used in a clean and tidy condition. Maintain a good working knowledge of Health and Safety procedures and fire precautions and operate correct procedures and participate in policy review and development and data collection where appropriate. Undertake any other duties appropriate to the grade and purpose of the job as may be agreed by the post holder. WORK SETTING AND REVIEW The Clinical Pharmacist will: Be able to work independently to achieve objectives with minimum supervision. Work flexibly to meet the needs of clients and to achieve objectives set by the line manager Participate in regular supervision sessions and appraisals with the line manager. Attend training and development activities as identified and participate in meetings as required.
