This is a fantastic opportunity for the right candidate to develop their professional skills and expertise in all areas of Practice Development by working closely with our pathway Matrons. The planned /community care team is made up of CMHTs, the specialist perinatal mental health team, Adult ADHD team, Acute Recovery team, The homeless team, AO, Community Forensic team, Maternal Mental Health team and the Mental Health Facilitators. The PDN will be working closely with the matrons that cover the specialist perinatal mental health team, Adult ADHD team, Acute Recovery team, The homeless team, Community Forensic team, Maternal Mental Health team and the Mental Health Facilitators to support them in improving the quality of care, safety and documentation within those teams. The community /planned care teams operate a 9-5 Monday to Friday service apart from the Acute recovery team that operates from 8am to 4pm. The plan is for the PDN to be directly managed by one of the matrons of those teams. The right candidate must be highly and self-motivated and able to work autonomously with some guidance from the matrons. Develop the objectives and standards of clinical practice required to support the aims of the service and establish systems to enable future monitoring / audit and to provide assurance. You will be a visible, accessible point of contact for staff, coordinating and leading on an agreed portfolio of responsibility within the directorate, which will include continual education and development of the staff group. Be expected to use professional clinical expertise to analyse and assess a wide range of data and qualitative information from across the directorate to establish current standards of clinical practice.
