As a Mental Health Practitioner (MHP)within Lancaster Medical Practice (LMP) you will play an important rolesupporting patients with mild to moderate mental health problems. You willprovide support to people suffering from various mental health conditions. Thework involves helping the patient to access the most appropriate services, torecover from their illness or to come to terms with it in order to lead apositive life. The role of the MHP will be part of the Advanced Clinical Teamwithin the Multi-Professional Clinical Team (MPCT) and you will work closelywith GPs, ACPs and other healthcare professionals.
Main duties of the job
As a MHP you'll be involved in:
Assessing and talkingto patients about their problems and discussing the best way to plan anddeliver their care within a time limited structure.
Ensuring the correct administration ofmedication and monitoring the results of treatment including reviewing andmaking recommendations in respect of anti- depressants and other commonly usedmedications.
Carrying out mentalhealth reviews for patients with a mental health condition or personalitydisorders.
Seeing patients onthe day, and for follow up in connection with their mental health concerns.
Telephone Triage of acute mental healthproblems.

Job responsibilities
Responding to distressed patients in a non-threatening manner andattempting to understand the source of distress and building relationships withpatients to encourage trust, while listening to and interpreting their needsand concerns.
Applying 'de-escalation' techniques to help people manage theiremotions and behaviour.
Maintaining patient records and producing care plans and riskassessments, including assessment of the physical health of patients.
Working with patients' families and carers, helping to educatethem and the patient about theirmental health problems.
Liaising with patients, relatives and fellow professionals in thecommunity treatment team.
Identifying whether and when patients are at risk of harmingthemselves or others.
Ensure good networksare established with partner organisations and signposting as appropriate.
Within the scope of your professional qualifications andexperience take part in in- houseCBT and other psychological therapies or take steps to develop this.
Provide support to other pathways such as opioid dependence, endof life, pain control, and chronic disease management.
