Sunflower House (SFH) is a well-established Tier 4 children’s mental health inpatient service and part of Alder Hey Children’s NHS Foundation Trust.
The service is looking to recruit a permanent Band 6 Team Coordinator.
The Team Co-Ordinator role will provide:
* Leadership and support to a group of clinical staff including nurses, healthcare support workers and trainees
* Young people up to age 13 presenting with complex and comorbid mental health difficulties
* Clinical and day to day operational experience and leadership across the shift
* Nurse in charge role
* Management of clinical staff
Main duties of the job
* Demonstrate ability to engage proactively with patients and their families/ carers/ significant others to build positive working alliances.
* Ability to work with assessed needs embracing the wishes of the patient within a Tier 4 specialist CAMHS inpatient setting.
* Able to make systematic and comprehensive assessment/re-assessment of patient need in conjunction with the wider multi-disciplinary team, employing appropriate assessment tools, and within the CPA framework enabling the development of planned Tier 4 inpatient care that incorporates all aspects of the patient’s needs.
* Able to facilitate assessment of Carer’s needs and implement appropriate support plans under the direction and support of the unit Social Worker.
* Demonstrates the skills and knowledge essential for developing nursing risk management plans, and timely review of the risk management plans, identifying opportunities for positive risk taking in conjunction with the patient, Wider MDT and patient’s carers/relatives/significant others.
* Has up to date knowledge and skills in relation to role and responsibilities of the practitioner under the Mental Health Act 1983 and its application in practice and to be able to support junior qualified staff in the same way.
* Actively engages with the Case manager under CPA to identify appropriate resources to support the patient’s care package within the hospital setting and within their local environment.
* Works to develop effective multidisciplinary relationships through skilled communication with the unit and the wider CAMHS network including NHSE as required.
* Shows ability to effectively evaluate plans of care in collaboration with the patient as an ongoing process and make recommendations for changes in the care package.
* Work as a Senior Nurse in a Unit area ensuring a safe and therapeutic environment for patients, their families and visitors.
* Applies relevant policies and statutory requirements within the unit and hospital framework including governance and communicates these requirements to the wider team.
* Respects and applies the requirements of equality and diversity, promoting and role modelling these across the team.
* Acts as a role model to promote a culture of learning development vision.
* Implements, monitors and evaluates therapeutic interventions within an overall care programme, enabling patients to develop coping strategies within the context of Positive Behavioural Support.
* Is named nurse for a small group of patients, acting as Associate nurse for colleagues when on duty, and is a regular shift leader. Who will be able to offer support and guidance to band 5 staff in the delivery and development of care.
* Work alongside the Case Manager, acting as an associate in the coordination of care with internal and external multi agency network including NHSE and the CCG where appropriate.
* Provides Clinical and managerial Supervision to a group of defined nursing staff.
* Adheres to all National and local standards, policies and procedures.
* Demonstrates expertise in analysing and reflecting upon their own clinical practice and that of others in all aspects of care giving to young people and their families/carers.
* Supports the Matron/Head of Service in developing sustainable services in line with national specifications, local implementation requirements and a set financial envelope.
* Promote and introduce systems to continuously improve quality of care, drawing on national and local guidance, and able to lead on key elements in particular CQC and QNIC requirements.
* Engages in the assessment of Children as part of the Units responsibility to NHSE gatekeeping responsibilities at times leading on key areas/cases in line with NHSE referral procedures and response times.
Person specification
Essential / Desirable
* Registered RMN/LD Nurse (Currently registered with the Nursing & Midwifery Council)
* Or registered Paediatric Nurse with additional training and experience within an Inpatient mental health setting
* Substantial post registration clinical, leadership and managerial experience
* Effective advanced communication skills and interpersonal skills.
* Evidence of appropriate continuing professional development activity to maintain up-to-date knowledge and on-going competence in all aspects of the role
* Evidence of working autonomously and as part of a team
* Proven ability to evaluate the safety and effectiveness of their own clinical practice
* Evidence of further professional and leadership training.
* Post registration qualification in CAMHs or equivalent
* Extensive experience of caring for children in MH Inpatient setting.
* Previous experience developing a nursing team
* IT literate.
