We are looking for a motivated & enthusiastic Band 6 Physiotherapist to join the Multi-Speciality Optimisation Team at St. James's Hospital in Leeds. The successful applicant will join the integrated Occupational Therapy and Physiotherapy team, covering wards designated for those patients who no longer have medical care needs, yet require further therapy intervention prior to discharge or have complex social needs. The team works together to assess physical, cognitive and social needs to achieve and facilitate the best outcome for patients when leaving hospital.
The successful applicant will be able to demonstrate in their application form and at interview relevant experience including skills in acute assessment, autonomous decision making and discharge planning. Applicants should be forward thinking with excellent communication, team working skills and practice patient-centred care. The post includes supervision of others.

Main duties of the job
There will be opportunity for this team to be involved with a number of local & national collaboratives including discharge, frailty, dementia and deconditioning. We are looking for applicants with leadership qualities who will relish the opportunities to contribute, facilitate, design and lead service developments, innovations and audit within the team. Job responsibilities
1. To perform physiotherapeutic assessment of patients with diverse presentations and complex physical and psychological conditions, to provide a diagnosis and develop and deliver an individualised treatment programme.
2. To be responsible for their own continuing professional and personal development.
3. To hold responsibility for own caseload and be responsible for a defined area of the service or a particular patient type, working without direct supervision. Access to advice and support from a senior physiotherapist is available if required, clinical work is not routinely evaluated.
4. To undertake all aspects of clinical duties as an autonomous practitioner within this specialist field.

Qualifications
* Degree or equivalent
Experience
* Significant experience working as a physiotherapist
* Working with a wide range of professionals including medical, nursing and therapy colleagues
Skills
* Ability to communicate effectively verbally and in writing
* Ability to prioritise, organise and allocate workload
Knowledge
* Insight into clinical speciality e.g. through academic study at post-graduate level or equivalent
* Insight into current professional issues and those influencing the NHS standards of professional practice
