We are looking for an experienced Practice Nurse to join our nursing team.
We are a multidisciplinary Practice that includes GPs, Clinical Pharmacists, Practice Nursing, Health Care Assistants, and Physiotherapists.
Main duties of the job
1. To be fully proficient in Chronic disease management, vaccinations and immunisations, cervical screening, contraceptive reviews, wound care including compression, as well as other general treatment room duties.
2. To provide support to junior members of the nursing team including healthcare assistants and will direct their work to ensure that the Practices provide high quality care.
3. You will oversee infection control standards and cold chain compliancy.

2. Oversee recall systems for chronic diseases, cervical screening, immunisations, and other key areas as relevant to Practice Nursing.
3. Delivery effective implementation of management of patients with Chronic Diseases as detailed by the Quality and Outcomes Framework, local quality schemes and national quality schemes.
4. Support the implementation of evidence-based guidelines in conjunction with the wider team.
5. Encourage the development of integrated services within Primary Care to include integrated practice-based teams and multidisciplinary working.
6. Ensure that all nurse-related Care Quality Commission evidence table indicators are compliant, demonstrating evidence, review, and audit cycle.
Job responsibilities
1. Participate in an annual appraisal.
2. Actively participate in clinical and practice meetings.
3. Develops clinical protocols for the surgery and the team.
4. Provide Chronic disease management for Diabetes, COPD, Asthma, and CHD/CVD.
5. Perform cervical screening and promote breast awareness.
6. Administer adult and paediatric injections.
7. Provide adult and child immunisations ensuring full achievement of targets.
8. Ensure safe storage, rotation, and disposal of vaccines and drugs.
9. Undertake venepuncture and electrocardiography.
10. Undertake wound care/ management / dressing techniques.
11. Undertake leg ulcer management, dopplers and compression bandaging.
12. Provide advice on lifestyle issues, assisting and supporting patients in changing their unhealthy behaviours and refer appropriately.
13. Participate in the smoking cessation service.
14. Deliver travel health care, including vaccinations and advice for patients.
15. Recognise the signs of child abuse, family violence, vulnerable adults, substance abuse, addictive behaviours, suicidal tendencies, and be aware of local, referral procedures.
16. Work with safeguarding teams where relevant.
17. Ensure that current evidence-based approaches to patient care are standard practice.
18. Apply infection control measures when collecting and handling of laboratory specimens, disposing of waste materials, dealing with blood and body fluids spillages and report and treat sharps injuries.
19. Undertake first aid and management of emergencies, e.g. burns, haemorrhages, hypo/hyperglycaemic emergencies/asthma attacks.
20. Provides a comprehensive family planning service.
21. Audit outcomes of care against standards and initiate change as necessary ensuring follow-up audit cycle.
22. Maintain and update disease registers.

Qualifications
* Nursing degree or other relevant nursing qualification granting professional license to practice primary care nursing.
* Qualified in chronic disease management for Diabetes, Asthma, COPD and CHD/CVD.
* Wound dressing and leg compressions.
* Cervical screening qualification.
* Venepuncture and electrocardiography.

* Previous experience of Care Quality Commission evidence table to achieve nursing-related indicators to a high standard.
* Stable career history showing clear commitment and progression.
* Independent prescribing qualification.
