Job Summary
This role entails providing person-centered assistance for people with complex needs and their carers while working within an integrated Health and Social Care team. The position holder will be responsible for ensuring adherence to legal frameworks, such as the Mental Capacity Act, the Care Act of 2014, the Deprivation of Liberty Safeguards (DoLS), and safeguarding procedures. Independence, avoiding hospitalization, facilitating prompt discharges, and providing high-quality interventions through a strengths-based approach are the main goals. In order to attain successful results, the position also calls for upholding professional standards, working with multidisciplinary teams, and contributing to service improvement.
Responsibilities
* Assessments and Planning: To support people and carers, carry out intricate evaluations, care plans, and reviews utilizing a person-centered, strengths-based methodology.
* Compliance and Legal Obligations: Comply with the Mental Capacity Act, Mental Health Act, Care Act, and Safeguarding procedures, which include preparing court reports and conducting Best Interest Assessments.
* Service Delivery: Manage timely hospital discharges while avoiding needless admissions, and encourage independence through personal networks, community services, and assistive technology.
* Risk management and safeguarding: Create risk assessments, handle complicated safeguarding cases, and put plans in place to protect adults who are at danger.
* Collaboration and Communication: Participate in multidisciplinary teams, interact with carers and service users, and disseminate knowledge to encourage self-sufficiency.
* Development and Leadership: Oversee projects aimed at improving services, uphold professional standards, and assist in achieving team performance goals.
* Resource Management: Properly distribute and oversee resources, such as individual spending plans and rehabilitation strategies.
Experience
* Extensive background in social work or related fields, encompassing intricate case management and statutory responsibilities.
* Skilled in implementing legal regulations (Care Act, Mental Capacity Act, and safeguarding).
* Experience working within multi-disciplinary and integrated health and social care environments.
* Strong analytical abilities and decision-making capabilities for managing complex cases.
* Excellent communication skills for engaging with service users, their families, and professional teams.
* Proficient in risk assessment, rehabilitation planning, and the use of assistive technologies.
* Comprehensive knowledge of legal and policy frameworks governing social care and health services.
* Specialized understanding of the Mental Capacity Act, Best Interest Assessments, and safeguarding protocols.
* Adaptable, proactive, and resilient in responding to changing service needs.
* Dedicated to upholding professional standards and fostering service enhancements.
