Job Summary
The Referrals and Assessment Manager plays a crucial role in overseeing the referral process and conducting assessments to ensure that clients receive appropriate services. This position requires a proactive individual who can educate staff and clients about available resources, facilitate communication between various stakeholders, and maintain high standards of service delivery. Duties
1. Manage the referral process, ensuring timely and accurate handling of client referrals.
2. Conduct comprehensive assessments to determine client needs and eligibility for services.
3. Educate staff on best practices for managing referrals and conducting assessments, fostering a culture of continuous improvement.
4. Collaborate with external agencies and stakeholders to streamline referral pathways and enhance service delivery.
5. Monitor and evaluate the effectiveness of referral processes, implementing improvements as necessary.
6. Maintain accurate records and documentation related to referrals and assessments in compliance with organisational policies.
7. Provide training and support to team members on assessment tools and techniques.
8. Address any issues or concerns raised by clients or staff regarding the referral process.
Experience
1. Proven experience in a managerial role within a social care or health-related environment is essential.
2. Strong background in conducting assessments and managing referral processes is highly desirable.
3. Excellent communication skills, with the ability to educate diverse audiences effectively.
4. Demonstrated ability to work collaboratively with multidisciplinary teams.
5. Proficient in using case management systems and maintaining accurate records.
