Use a range of verbal and non-verbal communication tools to communicate effectively with patients, carers, families and colleagues. Complete mandatory training. Participate in the review and appraisal process. Use clinical systems for record keeping, audit and evaluation. Provide support with digital transformation and offer IT/System support across our sites. Ensure all social media pages are up-to-date, informative and engaging. Build key relationships across PCN personnel and the wider team Provide coordination and navigation for people and their carers across health and care services, alongside Social Prescribing link workers and other primary care roles; Support the coordination and delivery of MDTs within PCNs as required; Focus upon patients and provide support necessary fulfil effective personalised care requirements; Help patients to manage their needs, answering their queries and supporting them to make appointments with a focus upon promoting patient independence and autonomy wherever possible; Coordination of key meetings (including Care Home residents and frail/vulnerable people living within their own homes.
