Lead Community Diabetes Specialist Nurse

An exciting opportunity has arisen for a Lead Community Diabetes Specialist Nurse to manage the community Diabetes service. The community Diabetes Nursing team provides a community-based specialist service for adults living with diabetes.
The successful candidate will share our commitment to patient-centered care. They will be highly experienced, with advanced skills in managing complex diabetes patients in a variety of settings. The post holder will manage a team of Diabetes Specialist Nurses to successfully deliver a specialist service to adults living with diabetes.
They will be able to make autonomous, evidence-based decisions and lead by example to educate and support colleagues in primary and secondary care. The post holder will work to proactively manage, coordinate, and support an identified caseload of patients with diabetes. This will include patient and carer education, annual health checks, and oversight of medication management.
The candidate will be an accessible and responsive team member with clinical responsibility for a defined patient group, able to make informed decisions within their own area and scope of professional practice to ensure agreed standards, objectives, and targets are met.
Main Duties of the Job
* Manage the multidisciplinary team of practitioners and the service administration support in a defined area.
* Provide operational leadership for the service.
* Take the lead responsibility for the provision of high-quality care in the service to the local community in accordance with departmental guidelines.
* Provide clinical leadership, taking the lead on specific professional initiatives in collaboration with other Diabetes team members.
* Be responsible for service development, particularly to develop integrated working and improved patient pathways across primary/secondary care services.
* Be responsible for the effective use of the budget for this service area, including the procurement of appropriate equipment and supplies.

Job Responsibilities
* Be responsible for the direct delivery of the diabetes service provided by the team.
* Ensure that the service is planned, developed, and delivered in line with national initiatives and priorities and policies.
* Ensure the service integrates with other teams across the Trust to achieve a coordinated and complimentary service provision for all patients referred.
* Investigate and implement new ways of working and role redesign in collaboration with the Modern Matron.
* Maintain service standards and documentation to meet all legal requirements.
* Be responsible for appropriate data collection for the service to meet both internal and external requirements.
