As part of our East Sussex community transformation plans, we are starting to develop our Neighbourhood Mental Health Teams. For Specialist Older Adult services, there will be 5 teams across East Sussex with a range of multidisciplinary professionals in each team who will provide assessment, treatment, and interventions including the provision of intensive support and treatment to enable people to receive care and treatment in their home setting and avoid the need for hospital admissions.
Main duties of the job
We have Band 4 roles, Registered Nurse Associate, who will provide intensive support and treatment to people in the community. These post holders will work alongside nurses and OTs and will be providing intensive support and interventions to patients in their own homes, preventing them from needing an admission to hospital. We would like you to join us and be at the forefront of leading the community transformation changes.
About us
In return, you will bring us commitment, enthusiasm, and the ability to work as part of an innovative service that is always looking for ways to improve the care it delivers. Joining the Trust, you would become part of a network of professional nursing and OT staff working with older adults and people with dementia (and their carers) and have the opportunity to network with, and learn from, colleagues in similar roles across the Trust.
Job responsibilities
In East Sussex, we are proud and excited to be working in partnership with local service providers across the health and social care system to provide high quality, innovative, and joined-up care for older people with mental health needs and/or dementia and their families.
