We are a dynamic, forward-thinking PCN team, who are passionate about developing and delivering excellent quality local services to meet the needs of our patients and reducing health inequalities.
We are looking to expand our established Pharmacy team by recruiting a Clinical Pharmacist to work alongside our practices and PCN staff.
We would like you to join our team as a Pharmacist to provide safe, quality, and cost-effective pharmacy services in primary care working with a wide range of colleagues within the Primary Care Network (e.g. Care Coordinators, Health and Well-being Coaches, Social Prescribing Link Workers, Nurse Associates, First Contact Physiotherapists, Mental Health Nurses, Occupational Therapists); alongside the wider team including the Practices, Community Pharmacy, the Voluntary and Community Sector, as well as statutory service providers including the CCG.
Main duties of the job
You will work as part of a multi-disciplinary team in a role that will mix service development work with a patient-facing role. You will need to be committed to improving health and putting patients first. Your ability to build effective relationships with patients, GPs, Nurses, Community pharmacists, Hospital and practice staff will aid you in this role, alongside your excellent communication skills.
You will be employed by Conexus Healthcare CIC.
This role may be based at any, or multiple practices within the PCN.

West Wakefield is a Primary Care Network of 5 practices; we are a network of forward-thinking General Practices covering circa 68,000 patients in the West Wakefield area.
Conexus Healthcare CIC is the Confederation for general practice in Wakefield, we have four areas of work:
1. Delivery of high-quality public-facing Health and Wellbeing services across the Wakefield District: We develop, manage and deliver health and care services that support the population of the whole Wakefield district.
2. Delivery of Training, Development and Consultancy support across the United Kingdom: We design and deliver a range of training courses, development activity and provide advice and guidance.
3. Supporting Our Practices and Primary Care Networks (PCNs): We support general practices and PCNs across the Wakefield district to be more resilient, sustainable and deliver better patient care.
4. Providing a voice and influence for General Practice in the Health and Care System: We provide a forum to develop a unified voice for General Practice and support general practices to cooperate and work collaboratively.
Built within primary care, for primary care, Conexus Healthcare CIC works with local GPs and their practices to champion investment in and deliver effective primary care at scale. By connecting in this way, we are stronger, more resilient and can care effectively for over 370,000 local people together.

Person Specification
Experience
* An appreciation of the nature of GPs and general practices.
* An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing.
* Excellent interpersonal, influencing and negotiating skills.
* Excellent written and verbal communication skills.
* Demonstrate the ability to communicate complex and sensitive information in an understandable form to a variety of audiences (e.g. patients).
* Able to obtain and analyse complex technical information.
* Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate.
* Able to work under pressure and to meet deadlines.
* Produce timely and informative reports.
* Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
* Work effectively independently and as a team member.
* Demonstrates accountability for delivering professional expertise and direct service provision.
* Minimum of 2 years post-qualification experience.
* In depth therapeutic and clinical knowledge and understanding of the principles of evidence-based healthcare.
* Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions.

Qualifications
* Mandatory registration with General Pharmaceutical Council.
* Membership of the Royal Pharmaceutical Society.
* A member of, or working towards, Faculty membership of the Royal Pharmaceutical Society.
* Specialist knowledge acquired through postgraduate diploma level or equivalent training/experience.
* Independent prescriber or working towards/intent of gaining independent prescribing qualification.
* CPPE approved or willing to gain this qualification.

£41,820 to £48,616 a year depending on experience and skills.
