Job overview
We are looking for a highly motivated and forward-thinking individual. You will have excellent organisational and people skills and the ability to work with and influence staff within and outside of the ICB.
You will operationally lead the ICS's Population Health Management Programme delivering on our priorities in line with our PHM Strategy which aligns with the ICS’s five-year joint forward plan.
Main duties of the job
* Provide comprehensive and timely project management aligned with ICS needs, managing the full lifecycle, reporting progress, and addressing risks and issues.
* Deliver significant projects to improve healthcare access, experience, and outcomes for patients and the public, utilising knowledge of population health management.
* Work with colleagues and subject matter experts on strategic planning, design, and delivery of projects, and strengthen links with teams across Norfolk & Waveney ICS.
* Demonstrate and present knowledge of Population Health management through interactions with Health & Care providers.
Person specification
Knowledge, Training and Experience
* Educated to degree level or equivalent level of experience of working at a senior level in a specialist area.
* Extensive knowledge from a variety of specialist subject areas.
* Evidence of relevant continuing professional development.
* Must have an understanding of the background to and aims of current healthcare policy and appreciate the implications of this on engagement.
* Experience of leading change projects.
* Experience of providing support and input into complex programmes.
* Experience of delivery of project management office functions.
* Experience of preparing briefing papers and correspondence at Board level.
* Demonstrate knowledge of Population Health management.
* Significant experience of managing projects and delivering them within agreed timescales and budgets.
Communication Skills
* Must be able to provide and receive highly complex, sensitive or contentious information, negotiate with senior stakeholders on difficult and controversial issues, and present complex and sensitive information to large and influential groups.
* Excellent communication and presentation skills with an ability to engage with a wide range of people at all levels within the organisation.

* Problem-solving skills and ability to respond to sudden unexpected demands.
* Ability to analyse complex facts and situations and develop a range of options.
Planning Skills
* Demonstrated capability to plan over short, medium and long-term timeframes and adjust plans and resource requirements accordingly.

Hybrid Working
