Community Psychiatric Nurse
MID Community Mental Health Services for Older People (CMHTOP) are seeking an experienced and enthusiastic community mental health nurse to join the team. The successful candidate will work within a community team based in the area, partnering with statutory and voluntary sectors to facilitate a whole system approach to care delivery. The Post Holder will act as Care Coordinator/Lead Professional, where appropriate, to patients requiring the skills of a Mental Health professional, providing a holistic, clinical autonomous nursing service to enable individuals to engage in the best standards of care available.
The candidate must have a full and valid driving licence and access to a vehicle for work purposes due to the community element of this role.
You will hold lead and professional responsibility for a number of service users, demonstrating excellent communication skills and the ability to work collaboratively with a multidisciplinary team. You will be committed to delivering a high-quality service, working both independently and as part of a well-established, innovative, and recovery-focused team in meeting the needs of service users and their carers.
In addition to holding a caseload, the successful applicant will offer initial assessments to clients referred to the service and support individuals in mental health within the community. There are various training opportunities available to build skills in the delivery of low-intensity psychological interventions.
