Care Coordinator
We are seeking a compassionate and experienced Supported Living Care Coordinator to oversee our supported living services. You will be responsible for ensuring the delivery of high-quality care and support to individuals with learning disabilities, autism, mental health conditions, and complex needs. This role involves managing staff, ensuring compliance with CQC regulations, and maintaining excellent service standards.
Key Responsibilities:
1. Oversee the day-to-day operations of supported living services, ensuring high standards of care.
2. Lead, support, and manage a team of support workers, providing supervision and training.
3. Ensure compliance with CQC regulations, health & safety, and safeguarding policies and supported living best practices.
4. Develop person-centred support plans that promote independence and well-being.
5. Manage staff rotas, recruitment, and performance to ensure effective service delivery.
6. Liaise with families, local authorities, and healthcare professionals.
7. Monitor budgets, expenses, and funding for service sustainability.
8. Conduct audits and implement improvements where needed.
Minimum Requirements:
1. Previous experience in supported living or domiciliary care management.
2. Level 5 Diploma in Leadership for Health & Social Care (or willingness to work towards it).
3. Excellent leadership, communication, and problem-solving skills.
4. Ability to work flexibly and handle emergency situations effectively.
You will be responsible for:
1. Maintaining the staff rotas and ensuring they have schedules for 1 month ahead.
2. Checking staff documents and ensuring all paperwork for each staff member are cross-checked and monthly audits are completed.
3. Producing monthly client invoices and ensuring they are paid on time.
4. Building professional relationships with local authorities while gaining business growth with their care packages and clients' assessments.
5. Monitoring and reporting any changes in our client's condition to the appropriate healthcare professionals, including completing paperwork and gathering information to upload to online systems.
6. Working in a way that ensures adherence to the CQC standards and keeping all company policies and procedures up to date.
7. Covering some care calls within this role to ensure all client and staff needs are met daily.
8. Being part of the on-call rota to support staff in their absence.
Desirable:
1. Experience in managing services for individuals with learning disabilities, autism, or mental health conditions.
2. Knowledge of local authority funding and safeguarding procedures.
