Specialist Palliative Care Clinical Nurse Specialist
We are delighted to offer this exciting opportunity for an experienced nurse to join our Specialist Palliative Care Team (SPCT), currently consisting of Hospital and Community Palliative Care Nurse Specialists and Doctors, Specialist Pharmacy support, Clerical support, and a Hospice at Home Team.
The SPCT provides specialist support, care, and advice to patients and their families living with life-limiting illness in East North Wales, across primary and secondary care.
As well as providing community Specialist Palliative Care support, the post holder will have designated in-reach to HMP Berwyn. They will work in partnership with the health and custodial teams, contributing to the delivery and service development of high-quality accessible palliative care, providing specialist support, care, and advice to those with life-limiting disease.
The post holder will be expected to work 5 days per week and regular weekends, during the hours of 9 - 5 pm.

Main Duties of the Job
The Highly Specialist Nurse is an autonomous practitioner and has the freedom to act to influence patient care and service provision. They will exercise higher levels of judgement, discretion, and decision-making in specialist palliative care, in relation to the management of patients, their carers, and professionals involved in their care, across all specialties and all care settings.
The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

* Substantial experience in a senior position
