As an occupational therapist working within the Adult Community Therapy team, you will have the opportunity to champion the key values of your profession.

Your caseload will be varied: you will have the opportunity to work alongside adults who are experiencing a range of long-term health conditions and acute deteriorations in their functional abilities. A key aspect of this will involve assessment of frailty and providing meaningful support for people with palliative health conditions.
We work closely with the wider multidisciplinary and multiagency teams to ensure that our patients receive high quality and equitable care.

* Degree or equivalent diploma in Occupational Therapy
* Current Health and Care Professions Council (HCPC) Registration
* Post graduate experience as an occupational therapist in rehabilitation with older people and/or long-term conditions and/or community/community hospitals/in-reach
* Evidence of leadership experience
Desirable
* Member of Royal College of Occupational Therapy
* Clinical Educators qualification
* Experience of working in community and community hospital settings
