Job summary The post holder will support the Trust and Integrated Care System's commitment to deliver the ambition of the national learning disabilities and autism programme (previously referred to as Transforming Care (TCP)) through ensuring people, adults, and children, are supported in the most appropriate setting to meet their needs. The role will have a focus on supporting people to discharge from specialist hospital placements into the community, or into the least restrictive environment, therefore reducing the number of people who have a learning disability and/or autism who are in specialist hospitals or support the MDT, thinking creatively around providing support within the community with the aim of avoiding admissions to Mental health hospitals. Main duties of the job The post holder will commission highly personalised care and support packages to enable people to live meaningful lives in the community in settings which are sustainable, high quality and safe, living as part of the community to allow people to thrive and live their best life. The Clinical Commissioner will utilise and adhere to nationally recognised tools including, the 12-point discharge plan, Care (Education) and Treatment Reviews (C(E)TRS), 6-8 week checks and locally agreed processes and protocols including provider brokerage and frameworks, housing protocols and funding panel processes, ensuring packages are personalised, high quality and financially effective. The post holder will also be experienced in the application of legal frameworks, care management approaches, Ministry of Justice restrictions, safeguarding and quality assurance. It is intended that the post holder will be allocated a caseload of people who reside within hospital. The base for this role is Wayside House in Coventry, and home working, there is a need to travel nationally.
